Viles in a laboratory wait to be tested in College Park, Maryland on September 15, 2011A US government laboratory mistakenly mixed a common flu strain with a dangerous and deadly type of bird flu and shipped it to another lab, the Centers for Disease Control said Friday. No one was exposed to the mixed flu strain, said CDC director Tom Frieden. "These events should never have happened," said Frieden at a press briefing. Frieden said he has issued a moratorium on the transfer of any biological samples, including infectious agents, within or outside the CDC until an investigation is complete.

FILE - This Nov. 15, 2012 file photo shows Gen. Carter Ham, then-head of the U.S. Africa Command, at the University of Sciences Politiques, in Paris. Two of the four U.S. deaths in Benghazi might have been prevented if military leaders knew more about the intensity of the sporadic gunfire directed at the CIA facility where Americans had taken refuge and they had pressed to get a rescue team there faster. In closed-door, congressional testimony, now-retired Gen. Carter Ham and other top military commanders described the chaotic night of Sept. 11, 2012, in which they thought Americans were safe, not facing sometimes sustained gunfire throughout the night, and were focused on a potential hostage situation when, in fact, Ambassador Chris Stevens was already dead. (AP Photo/Thibault Camus, File)WASHINGTON (AP) — Two of the four U.S. deaths in Benghazi might have been prevented, military leaders say, if commanders had known more about the intensity of the sporadic gunfire directed at the CIA facility where Americans had taken refuge and had pressed to get a rescue team there faster.
